Hopkins Restorative Care Center is looking for a Licensed Social Worker to join our team and play a key role in supporting residents and families from admission through discharge
.

With our newly refreshed TCU
, you’ll work closely with short-term rehab residents, families, nursing, therapy, and facility leadership to coordinate services, remove barriers, support care planning, and help create smooth, successful transitions.

• Build strong relationships with residents and families

• Enjoy discharge planning and finding solutions

• Communicate clearly and work well across departments

• Stay organized in a fast-paced environment

• Want to be part of a leadership team that values teamwork and accountability

Minnesota Social Work license required.
Skilled nursing or long-term care experience is preferred.
