Primary Responsibilities

This employee will provide case management services to adults with a diagnosis of a severe mental illness.

  • (1) Provide intensive case management to identified clients.
  • (2) Have ongoing contact with community agencies necessary to develop additional resources and coordinate services.
  • (3) Assist in developing and accessing housing for clients.
  • (4) Must be able to establish rapport with clients and families.
  • (5) Must have transportation and be willing to assist clients by providing outings and keeping scheduled appointments with various community advocacy agencies.
  • (6) Employee is also responsible for timely record keeping associated with the services provided.
  • (7) May fill in from time to time during the absence of other staff, as assigned by supervisor.
  • (8) Attend one (1) individual and one (1) group supervision monthly as scheduled by supervisor.
  • (9) Complete and maintain documentation of a minimum of six (6) hours of training or continuing education each year.
  • (10) Other duties as assigned/required.
Qualifications

Employee is required to have at least a bachelor's degree in psychology, social work, or related field with at least 1 year of experience in working in a human services setting. Must have a valid driver's license, reliable transportation, and be willing to transport clients on a daily basis. Must present proof of automobile insurance and pass a Criminal Record Check.

Within 6 months of employment, this employee must attend the Core and SMI case management trainings.
