Primary Care Triage Nurse LPN (Full Time)

Join Jennie Stuart Health as a Primary Care Triage Nurse (LPN) where you will perform care coordination and professional nursing functions to assess the severity of patient health concerns using protocols and algorithms. You will determine appropriate workflows for emergency care or clinic appointments within the scope of a practical nurse license.

Position Type: Full-Time/Regular

Customer Services:

• Communicate clearly and calmly with callers.
• Maintain communication between medical staff, administrative staff, and patient/families.

• Manage nurse‑directed patient telephone calls requesting health care services.
• Evaluate patients’ health‑care needs based on signs and symptoms using triage protocols, guidelines, and standing orders.
• Obtain appropriate input and direction from the provider as needed to determine best clinical course and recommendations for specific patient health‑care needs.
• Accurately document symptoms/complaints, nursing assessment, advice provided, and patient/caller response.
• Coordinate and schedule same‑day appointments on the provider’s schedule.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease, preventive care, and family health.
• Ensure pertinent medical information is forwarded to proper departments for continuity of patient care.

• Contribute to the overall cleanliness and appearance of personal workspace and department.

• Education: Graduate of an accredited nursing school.
• Experience: Previous experience in medical practice is preferred.
• License / Certification: Current Kentucky or Multi‑State license;
  Basic Life Support certification.
