- Communicate with patients, families, and clinicians to confirm appointments
- Maintain accurate records in the scheduling system and electronic health records
- Monitor and adjust schedules to accommodate changes in patient condition or staff availability
- Ensure compliance with agency policies, state regulations, and payer requirements
- Provide excellent customer service and timely follow-up on inquiries
- Previous experience in healthcare scheduling, home health or hospice strongly preferred
- Strong organizational and time-management skills
- Proficient in scheduling software and EHR systems
- Excellent communication and problem-solving abilities
