As an Advanced Nurse Practitioner within the Acute Winter Hub, you will have the opportunity to deliver high-quality patient care in an innovative remote service, work alongside experienced multidisciplinary colleagues and contribute to the continued development and improvement of care for patients across the local healthcare system.

We have a number of remote shifts available during service hours 09:00 - 17:00 Monday to Friday. Shifts are available on a remote basis, though regular travel to the office will be required for team meetings.

Main duties of the job

As the successful postholder, you will be an experienced healthcare practitioner with a background in acute and urgent care. Ideally, you will have completed an MSc in Advanced Practice and be an independent prescriber. You will be able to demonstrate a clinical portfolio and relevant competencies in line with the HEE National Framework for Advanced Clinical Practice. As an Advanced Clinical Practitioner, you will work across the four pillars of advanced practice: clinical practice, leadership, research and education.

Working within our remote clinical services, you will practise as an Advanced Nurse Practitioner, supporting the Acute Winter Hub.

In this role, you will autonomously assess and treat patients presenting with both acute and chronic conditions, including differentiated and undifferentiated presentations. You will determine clinical diagnoses, develop treatment plans and arrange appropriate discharge or referral pathways while maintaining accurate patient records and ensuring patient safety.

You will act as a source of clinical expertise for less experienced colleagues, helping to develop skills and knowledge across the team while working collaboratively within a multidisciplinary environment.

You will play a key role in ensuring a high-quality, patient-centred service is delivered across all remote clinic settings.
