If you want to be more than just a name on a rota and instead become a trusted, embedded clinician within a practice team, this role offers exactly that. Here, you will build meaningful relationships with patients, influence prescribing at surgery level and see the impact of your clinical decisions every single day.
At School Lane Surgery, our Practice Clinical Pharmacists play a central role in how we care for patients. This is not a peripheral position. You will be at the heart of the practice, combining direct patient care with leadership in medicines optimisation and quality improvement.
You will take an active role in chronic disease management, lead clinical medication reviews and help patients gain confidence in their treatment. The work is varied and sometimes fast paced, but it is deeply rewarding. We work side by side, share challenges and celebrate successes together. Your expertise will be valued, your opinion listened to, and your contribution recognised.
This is an opportunity to practise confidently, grow as a clinical leader and help shape how we deliver care now and in the future.
Main duties of the jobYou will help patients achieve the best possible outcomes from their medicines by:
- Switching medications in line with agreed and approved protocols
- Improving repeat prescribing processes within General Practice, including promoting repeat dispensing and online ordering
- Minimising clinical risk and reducing medicines waste
Working in a patient-facing capacity as part of a multidisciplinary team, you will clinically assess and treat patients using your extensive experience and in-depth knowledge. You will take direct responsibility for the care and prescribing of patients.
You will:
- Deliver and further develop pharmacy-led medicines optimisation across the practice and wider multidisciplinary team
- Lead, promote, support and facilitate medicines optimisation within your specialist area, ensuring best value, reduced harm and improved patient outcomes
- Lead and deliver evaluation and audit of service delivery within your specialist area to ensure high quality provision and continuous improvement
- Work in line with the principles of Medicines Optimisation and relevant up-to-date national guidelines and evidence
- Support the Prescribing Lead and other colleagues across the practice
We care for a diverse and growing population of around 17,500 patients across two well-established clinical sites and a dedicated administrative base. Our scale brings breadth and variety to the clinical work, yet we have worked hard to maintain the feel of a close, supportive and approachable team.
At School Lane, our ethos is simple. Every role matters and every voice counts. We share knowledge openly, support each other through the pressures of primary care and remain focused on delivering care that is safe, effective and genuinely centred around our patients.
We are a busy practice, but that energy brings purpose and satisfaction. You will be trusted to take ownership of your role, supported when challenges arise and encouraged to develop your interests and skills.
There are real opportunities here to shape services and make a lasting difference.
For the avoidance of doubt, this is a surgery-based role. Whilst School Lane Surgery has a Pharmacy within the building, this is operated by another company, so it is not related to this role.
