Practice Clinical Pharmacist

At School Lane Surgery, our Practice Clinical Pharmacists play a central role in how we care for patients. This is not a peripheral position. You will be at the heart of the practice, combining direct patient care with leadership in medicines optimisation and quality improvement.

You will take an active role in chronic disease management, lead clinical medication reviews and help patients gain confidence in their treatment. You will help patients achieve the best possible outcomes from their medicines by:

• Switching medications in line with agreed and approved protocols
• Improving repeat prescribing processes within General Practice, including promoting repeat dispensing and online ordering
• Minimising clinical risk and reducing medicines waste

Working in a patient-facing capacity as part of a multidisciplinary team, you will clinically assess and treat patients using your extensive experience and in-depth knowledge. You will take direct responsibility for the care and prescribing of patients.

You will:

• Deliver and further develop pharmacy-led medicines optimisation across the practice and wider multidisciplinary team
• Lead, promote, support and facilitate medicines optimisation within your specialist area, ensuring best value, reduced harm and improved patient outcomes
• Lead and deliver evaluation and audit of service delivery within your specialist area to ensure high quality provision and continuous improvement
• Work in line with the principles of Medicines Optimisation and relevant up-to-date national guidelines and evidence
• Support the Prescribing Lead and other colleagues across the practice

We care for a diverse and growing population of around 17,500 patients across two well-established clinical sites and a dedicated administrative base. As Practice Clinical Pharmacist, you will:

• Work as part of a multidisciplinary team in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas
• Provide pharmaceutical consultations to patients with long-term conditions as an integral member of the team
• Offer proactive leadership on medicines and prescribing systems to colleagues, patients and carers
• Improve patient and carer understanding of and confidence in their medication, supporting safe and effective use
• Provide clear and timely advice in response to medication-related queries from patients and staff
