Care Coordinator (Level
2) – Learning Disabilities Team
We are seeking to recruit a Care Coordinator (level
2) for our Learning Disabilities Team who will deliver exceptional standards of person-centred care and support to customers.

Your delivery of personalised care and support will have the individual at its centre, promoting respect, dignity, engagement, and participation for our customers. ResponsibilitiesYou will be providing support to customers promoting and developing their life, health, and wellbeing. This may include:
- Helping with preparation of meals and drinks.
- Support with domestic household tasks.
- Administer and record medication.
- Support with money management.
- Encouraging community trips and facilitating customers participation.
- Support with household and personal shopping.
- Support with meeting friends and family.
- Support with high standards of nutrition.
- Adhere to health and safety legislation.
- Cleanliness and intimate personal hygiene.
- Opportunities for employment or education.
- Providing emotional support - Using active support techniques and positive behaviour support techniques to achieve set outcomes (training will be provided).
- Supporting customer to plan holidays and supporting them on holiday if needed (additional payment).
These principal responsibilities are not an exhaustive list of duties; they are only a general guide to the role. In consultation with the post holder, duties may change from time to time to reflect the changing needs of the customer/service.

- A starting salary of £28,598–£31,022 (this will be pro rata if working less than 37 hours per week).
