Summary
The Community Health Worker will function as a member of a multidisciplinary team that conducts participant support to help postpartum moms who use Women, Infant, and Children (WIC) services achieve good health practices around diabetes prevention. Primary responsibilities include enrolling participants, health coaching, data collection, collaboration with other team members to coordinate schedules for recruitment and follow-up across WIC clinics, and coordination with community partners.

Required Knowledge, Skills, And Abilities

  • Knowledge of community resources and experience working with community-based organizations preferred.
  • Proficiency in Microsoft Office Word, Access, Excel, electronic medical records (EMR), and other software programs required.
  • Ability to travel most days to WIC clinics (Thibodaux, Houma, Morgan City, Reserve, Luling, and other regional clinics).
  • Act as a health coach, leading group sessions on diabetes prevention to promote healthy behaviors.
  • Assist participants with navigating the healthcare system and processes.
  • Facilitate referrals for health information and services as recommended.
  • Communicate effectively with diverse populations about cardiovascular health risks in a culturally competent manner.
  • Advocate for participants to ensure program compliance.
  • Provide participants with community resources as needed.
  • Maintain accurate data and files for participants and program reporting.
  • Support research data collection from study participants post-training.
  • Support planning and coordination of team schedules across WIC clinics.
  • Perform other duties as assigned.

Background Check, Physical, and Drug Screening

Selected candidates must pass a background check and occupational health screening. A drug screening may also be required, conducted after a conditional employment offer.

Required Education and Experience

  • High School Diploma or GED required.
  • At least three years of health/clinical experience or equivalent health-related education required.
