• Working closely with healthcare professionals, housing providers, probation services, and other partner agencies.
• Completing risk assessments and safeguarding interventions where required.
• Building positive relationships with service users, empowering them to achieve their recovery goals.
• Contributing to multidisciplinary meetings to ensure coordinated, person-centred support.
• Promoting long-term wellbeing through effective care planning and regular reviews.

• Qualified Social Worker.
• Current registration with Social Work England (SWE).
• Experience working within Adult Social Care, Substance Misuse, Mental Health, or a related service.
• Confident in completing assessments, care planning, and risk management.
• Strong understanding of safeguarding legislation and person-centred practice.
• Evidence of continued professional development.
• Full UK driving licence.
