Key Responsibilities
  • Engage in high-impact patient care, focusing on individuals with chronic illnesses and promoting health & wellness.
  • Participate in innovative patient visits, including transitions of care and shared visits.
  • Work collaboratively in a dynamic team environment focused on continuous improvement.
  • Contribute to mission-driven initiatives that prioritize community health and well-being.
  • Deliver pharmaceutical care through direct patient interactions and digital platforms.
  • Conduct both face-to-face and virtual care sessions to manage chronic disease medications.
  • Analyze comprehensive medical and medication histories to ensure optimal patient care.
  • Reconcile medications during ambulatory care visits and perform necessary physical assessments.
  • Document clinical encounters in the electronic medical record system with precision.
  • Design and implement therapeutic plans that maximize efficacy and minimize toxicity.
  • Initiate, modify, and discontinue medication therapy based on approved protocols.
  • Order laboratory tests to monitor medication effectiveness and safety.
  • Report adverse drug events and medication allergies in compliance with procedures.
  • Provide thorough drug education to patients, families, and healthcare providers.
  • Assess and enhance medication adherence during counseling sessions.
  • Serve as a mentor and preceptor to pharmacy students and residents.
  • Collaborate seamlessly within and beyond the department, using agile methodologies.
Minimum Qualifications
  • Pharm

    D or B.S. from an accredited college/university of pharmacy.
  • Completion of an ASHP accredited PGY-1 Pharmacy Residency.
  • Valid Texas Board of Pharmacy license.
  • Certification in Basic Life Support (BLS), CPR, and First Aid.
  • Preferably a Board-Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP).
  • Minimum 2 years of community pharmacy experience and medication adherence counseling.
  • Experience in HIV/AIDS, Geriatrics, and Pediatrics is advantageous.
  • Completion of PGY-2 Pharmacy Residency in relevant fields is desirable.
  • Certification in immunization delivery or smoking cessation.
  • Proficient with Windows, Microsoft Word, and Excel.
  • Exceptional communication and customer service skills.
  • Ability to manage confidential information with discretion and security.
