Overview

Cognito LLC, in partnership with Planned Systems International, is recruiting a Medical Records Technician to support the ICE Health Services Corps (IHSC) at the Houston Contract Detention Facility. The role involves maintaining confidential, accurate electronic health records and ensuring high‑quality patient care within a 24/7/365 detention setting.

Job Responsibilities
  • Prepare, update, and maintain a medical record for each patient, ensuring accuracy of information.
  • Maintain the appointment system for patients and clinical staff.
  • Track compliance with internally scheduled appointments, sending timely reminders to IHSC staff.
  • File or scan laboratory, radiology, and other reports into the electronic medical record within prescribed timelines.
  • Route clinical reports to appropriate clinic staff within prescribed timelines.
  • Archive clinical information from the medical record in accordance with IHSC policy and procedures.
  • Review all documentation for completeness; route incomplete items to the provider for correction before recording.
  • Use multiple systems to process narrative and tabular material for record creation and maintenance.
  • Perform record‑keeping functions per program policies and position requirements.
  • Maintain proficiency with electronic health records.
  • Complete and pass Medical Records Technician competencies initially and annually.
  • Complete all initial, annual and ad‑hoc training as required.
  • Collaborate with the team to analyze protocols and recommend improvements.
  • Maintain patient confidentiality in compliance with the Privacy Act and HIPAA regulations.
  • Adhere to IHSC policies, procedures, directives, operational memoranda and accreditation standards.
  • Participate in IHSC programs such as Safety, Prevention, Infection Control, Quality Improvement, and Patient Education.
  • Attend and participate in general/medical staff meetings.

Full‑time position requiring availability during workdays, evenings, weekends, and holidays. Night shifts may be requested based on mission needs; on‑call availability is not required.

Minimum Requirements
  • Education: High School Diploma or GED equivalent.
  • Certification: Heartsaver CPR AED certification via the American Heart Association.
  • Experience:
    • Minimum 1 year in a healthcare setting as a Medical Record Technician, Unit Clerk, or similar role involving electronic medical record processing.
    • Minimum 1 year of proficiency in Microsoft Office programs (Word, Excel, Outlook, SharePoint).
  • Credentialing: Ability to pass a background check, drug screen, and meet physical and immunization requirements.
  • Knowledge of electronic medical record procedures, workflow, and documentation standards.
  • Understanding of medical terminology and HIPAA/Privacy Act regulations.
  • Ability to manage high-volume records, including intake, discharge, and external requests.
  • Skillful identification of documentation inconsistencies and resolution of discrepancies.
  • Ability to adapt to sudden schedule changes and work in a multicultural environment.
  • Proficiency in English (verbal and written).
  • Comfortable using computers, scanners, and printers for approximately 90% of the time.
  • Ability to utilize telephonic interpreter/translation services when necessary.
Physical Demands
  • Long periods of standing and movement over uneven surfaces.
  • Regular bending, crouching, stooping, stretching, and reaching.
  • Occasional lifting of moderately heavy items or patients.
  • Respond promptly (within four minutes) to emergency situations, including CPR and emergency care.
  • Assist with evacuations of detainees or staff during emergencies.
