Healthcare-For-the-Homeless--houston is seeking a dedicated professional to provide intensive case management for consumers with physical/mental health needs and substance use issues. You will coordinate access to housing, benefits, referrals, and transportation, while advocating with medical and social services partners.
You will develop negotiated care plans, monitor progress, and document outcomes in a multi-disciplinary setting, adhering to confidentiality and organizational policies.
