Behavioral Hospital of Bellaire, part of Universal Health Services, is seeking a Step-Down Navigator to meet with eligible patients and explain step-down programming as part of the continuum of care. Responsibilities include coordinating with Admissions, Clinical Services, Utilization Management, and other departments; collecting and entering data on eligibility, referrals, and treatment plans; and assisting leadership to identify trends and expand programs.
Hours:
Mon–Fri, 8:00 AM–5:00 PM.
