As a Behavioral Health Consultant, you’ll bring compassionate, timely behavioral health support directly into the primary care setting. Your clinical expertise will help adults, adolescents, and children navigate biopsychosocial stressors through integrated behavioral health screenings, brief interventions, clinical evaluations, and counseling.

• Identify behavioral health needs early through screening and psychosocial assessment in the primary care setting.
• Provide brief interventions, crisis intervention, counseling, and referrals that connect clients with timely support.
• Support children, adolescents, and adults experiencing behavioral health concerns and biopsychosocial stressors.
• Collaborate with clients, physicians, healthcare providers, and community agencies to coordinate integrated care.
• Strengthen the broader healthcare team through your clinical insight and client-centered approach.

• Receive ongoing case consultation with a clinical supervisor at least monthly.
• Work alongside a multidisciplinary team committed to compassionate, coordinated care.
• Participate in service coordination, follow-up, and prevention efforts that support long-term wellness.
• Help clients access voluntary family planning services and related preventive health services in compliance with funding requirements and state and federal laws, rules, and regulations.

• Screen new clients identified at risk in the primary care setting to identify presenting problems, address urgent issues and/or provide immediate referral as indicated.
• Administer psychosocial assessments and crisis intervention.
• Provide individual brief interventions to adults, adolescents, and children experiencing biopsychosocial stressors.
• Provide direct clinical services including clinical evaluations and counseling using motivational interviewing and brief intervention methods.
• Review client records.
• Develop and write record notes and evaluation reports.
• Assist with service coordination and follow-up procedures.
• Coordinate consultation and services to various community agencies and health services providers.
• Collaborate with clients, physicians, and other providers to develop and implement client-centered interventions and treatment plans.
• Re-assess clients and complete new treatment plans as indicated.
• Work a full-time flexible schedule that will include evenings and some weekends.
• Meet with clinical supervisor at least monthly to review client cases.
• Document all encounters with clients, including but not limited to all contacts and attempted contacts with or on behalf of clients in the client’s medical record within 3 working days of their occurrence.
