Lister in Houten Castellum zoekt een (senior) persoonlijk begeleider voor 28-32 uur per week. Je bent het eerste aanspreekpunt en verantwoordelijk voor de zorgverlening van de cliënten met autisme en psychiatrische kwetsbaarheden, gericht op ontwikkeling en zelfredzaamheid.
Je coördineert de zorg rondom de cliënt en werkt nauw samen met het sociale- en zorgnetwerk. Samen met collega’s zorg je voor een methodische begeleiding zodat cliënten zich optimaal ontwikkelen en zoveel mogelijk zelfstandig
