Position Purpose

A Community Outpatient Pharmacist provides pharmaceutical care for the outpatient populations according to professional standards and statutory requirements, directs pharmacy technicians, students, and interns, supports clinical care, medication preparation and distribution, offers education, consultations, immunizations, medication therapy management to improve patient outcomes, and engages in professional activities to improve public health awareness.

  • Broad knowledge of medications, compounding, and dispensing.
  • Thorough knowledge of pathophysiology, pharmacokinetics, and therapeutics.
  • Interpersonal skills to communicate effectively with a wide variety of hospital healthcare personnel and patient population.
  • General understanding of computers, including general operations and data retrieval.
  • Communication skills for clear written and oral communication.
  • Excellent customer service orientation skills to deal effectively with hospital personnel, outside customers, and community groups.
  • Training and skill to administer immunizations.
  • Maintain a working knowledge of applicable Federal, State, and Local laws and regulations.
Responsibilities
  • Interpret and evaluate prescriber’s medication orders to ensure appropriate drug therapy for each patient in accordance with legal and regulatory policies, rules, and standards.
  • Review, maintain, and screen patient medication orders and profiles for allergies, drug‑drug interactions, drug‑food interactions, contraindications, potential adverse drug reactions, therapeutic duplication, and out‑of‑range doses.
  • Communicate with physicians, nurses, and other healthcare providers and patients to intervene on orders that are unclear, incomplete, inappropriate, or could cause drug misadventure; recommend alternative therapy; document intervention activity.
  • Perform patient medication histories and counseling when necessary or requested; provide education materials as required.
  • Consult with and advise physicians and other health‑care professionals in developing safe, rational, and cost‑effective patient medication care plans.
  • Support and conduct oneself in a manner consistent with customer service expectations and Trinity Core Values.
  • Direct and monitor pharmacy technicians, interns, and students in preparing, packaging, labeling, and distributing medication.
  • Compound and dispense basic medications; ensuring that compounding/dispensing errors are minimized.
  • Initiate, direct, coordinate, and perform specific departmental projects.
  • Participate in teaching programs affiliated with the Department of Pharmacy, and training programs for nursing, physicians, pharmacists, and other allied health professionals.
  • Assist in the training and orientation of new pharmacists, pharmacy technicians, and pharmacy interns.
  • Administer vaccinations to patients following established pharmacy policy.
  • Maintain control and accountability for disposition of Controlled Substances, dangerous or toxic drugs, and investigational drugs as required by Federal and State regulations; properly maintain all legally required records.
