MDS Coordinator - Laurels of Huber Heights

Overview

The Laurels of Huber Heights is looking for an experienced MDS Coordinator to oversee the RAI process, conduct assessments, and coordinate care plans for residents.

• Complete MDS, CAA, and care plans within regulated time frames
• Coordinate scheduling of the RAI process with the interdisciplinary team
• Assess residents through physical assessment, interview, and chart review
• Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
• Review information from hospital, consults, and outside agencies to complete assessment and care planning
• Coordinate, identify, and/or initiate significant change in MDS
• Maintain MDS assessments per Medicare schedule and preserve PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remain current with American Association of Nursing Assessment Coordinators (AANAC) requirements

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure
• AANC certification a plus (RAC‑CT)
• Knowledge of the Resident Assessment Instrument (RAI) process, including PPS principles, strongly preferred
• Experience as an MDS Nurse
