Transition Lead – The Pivot Academy

Hours:

35.5 per week, term time plus 10 days. Permanent. Salary: £21,494.26 (starting salary) £12.75 per hour.

Opportunity to transform the lives of young people with complex needs. The Pivot Academy provides therapeutic nurture for learners with complex social, emotional, mental health and other associated needs. You will play an essential role in supporting high‑needs learners in transition.

• Collaborate with the SENCO, Pastoral & Welfare Lead, learners, families and external professionals to formulate, agree and implement personalised transition plans for learners moving into or out of Pivot.
• Ensure that positive and robust links are established and maintained between home and school, making the transition process as seamless as possible.
• Conduct visits to learners in other settings and homes to support smooth transitions.
• Maintain accurate records and report on transition progress.
• Work across the local area and travel to other settings – access to a car is essential.

Relevant experience in working with children and young people with complex needs, including safeguarding and welfare. Experience in developing and delivering transition plans is desirable. Full DBS and experience of working with vulnerable populations is essential.
