Working in Social Care is challenging and rewarding. You will be part of a busy team working closely with the multidisciplinary team to facilitate timely and safe discharges from hospital. The role supports adults and carers to deliver self‑directed support and self‑assessment, safeguarding, and a positive approach to risk.
Key Responsibilities- Facilitate timely and safe discharge of patients from hospital in partnership with multidisciplinary teams.
- Provide self‑directed support and self‑assessment to adults and carers.
- Safeguard individuals and adopt a positive approach to risk management.
- Deliver varied, challenging work ensuring individuals achieve personal outcomes.
- Travel is essential: use a car or council vehicle; a full and valid driving licence is required.
- Maintain professional development and enhance skills through training and practice.
- Level 2 or Level 3 Social Worker qualification.
- Experience in hospital social work preferred.
- Valid driving licence and ability to travel.
- Duty of a DBS check at the appropriate level.
