Lead Support Worker

Pay 13.21 per hour (25838 per annum) plus health cash plan and instant pay access with Stream

Looking for a role where you can help people turn their humble ones. As our Lead Support Worker (known by us as Support Coordinator) youll support customers with complex needs in their homes and out in the community.

Youll be part of a central team that steps in when services need cover so no two days are the same. If youre ready to leave behind the same old routine and do something that matters this could be your next move.

• Create person-centred support plans and lead a small team to deliver them
• Support customers with mental health substance use homelessness and isolation
• Help people build daily living skills and confidence in their community
• Carry out risk assessments reviews and safeguarding checks
• Set up and end tenancies making sure customers feel safe and supported

• Experience creating support plans and leading others to deliver them
• A calm resilient approach and confidence working on your own
• Skills to assess referrals and coordinate support for complex needs
• A passion for helping people live their best life

• Youll work Monday to Sunday on a rota-based system
• Youll work 37.5 hours per week on a rota-based system
• Youll cover a patch across services in West Yorkshire
• You need a vehicle insured for business use and we pay your mileage
• You need an Enhanced with barring list DBS check that we pay for

• 34 days leave (including bank hols and a me day to use for whatever you fancy) increasing to 39
