37.5 hours per week (part time considered)

Are you passionate about delivering high‑quality care in the community? Locala is inviting applications from Healthcare Assistants who share our commitment to person‑centred care and want to make a real difference to patients’ lives.

As part of our Community Nursing Network, you’ll work alongside qualified nurses to provide holistic, seamless care for housebound patients. Your role will include:

• Supporting nurses with assessments and care planning
• Delivering nursing‑based care to prevent unnecessary hospital admissions
• Promoting self‑care for patients with long‑term conditions
• Contributing to health promotion and innovative service improvement projects

We have nine teams across Kirklees, offering flexibility to support work‑life balance. Our service operates between 8:00 am and 9:00 pm, with core hours from 8:00 am to 5:00 pm. You’ll be expected to work one weekend in four and two late shifts (1:00 pm – 9:00 pm) over a four‑week period. Car driver essential.

The post holder will be expected to be able to travel independently across the borough.

This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.

Position may be subject to DBS check.
