• Assist with the implementation of a Case Management Program.
• Provide leadership and education to other members of the Case Management team.
• Develop safe discharge planning strategies.
• Communicate effectively with administrative and professional staff.
• Make decisions about discharge planning, home health situations, and community services.
• Understand Managed Care/HMO requirements, laws, and regulations.
• Navigate Medicare/Medicaid requirements and community health policies.

• Education:
  
  BSN required
• Experience:
  
  Minimum two years experience in Case Management preferred
• Licenses &
  
  Certifications:
  
  Current New York State RN license required;
  Certified Case Manager (CCM) highly desirable
