We are looking for a Home Visiting UCP/ACP to join our team, to cover maternity leave, on an ad hoc basis. This is a combination of visiting patients in their home with site attendance required as part of the role. This is far more than a traditional clinical role, as an autonomous practitioner, you will assess, diagnose, treat, and manage patients presenting with a wide range of urgent and acute conditions while working collaboratively with multidisciplinary teams to deliver exceptional patient care.

Alongside providing expert clinical care, you will play an active role in service development, quality improvement, education, and the implementation of innovative ways of working that enhance patient outcomes across Primary Care.

Main duties of the job

This is a dynamic advanced clinical role requiring excellent assessment, diagnostic, and decision-making skills. You will provide safe, evidence-based, person-centred care while working autonomously and collaboratively across multidisciplinary teams.

You will assess patients presenting with urgent and acute conditions, formulate personalised management plans, prescribe where appropriate, and ensure patients receive the right care at the right time.

Alongside clinical practice, you will contribute to service development, quality improvement initiatives, clinical leadership, and the continuous improvement of patient care across the division.

You will need excellent clinical judgement, adaptability, and a commitment to delivering outstanding patient care.
