NHS is seeking a Social Prescriber for Greengates Primary Care Network to improve health and wellbeing in Cottingham. You'll work with the YOURhealth team, helping individuals with social, emotional, and practical needs, empowering them to enhance their wellbeing.
The position requires excellent communication skills and an understanding of the biopsychosocial model. Your role will involve community outreach and occasional evening work. An enhanced leave package and health support are included in benefits.
