Clinical Reconnect Practitioner – Humberside
We have an exciting opportunity for a Clinical Reconnect Practitioner to join our team in Humberside! This is an opportunity you don't want to miss.
Job SummarySalary: £43,609 to £49,175 depending on experience. Full‑time.
Hours:
Monday – Friday, 9am to 5pm.
Location:
Humberside Region & Prison sites (HMP Hull & HMP Humber). Candidates are required to have a legal right to work in the UK; offers of employment will be subject to additional vetting and security checks.

- Support from the wider team
- Work autonomously in prison, community and police detention cells to identify and respond to requests for screening and assessment of people with identified health needs leaving the prison establishment within a 12‑week period or less.
- Take referrals for service users leaving the prison establishment within 12 weeks up to 28 days post‑release; aim to reduce reoffending by addressing health and social needs and ensuring onward referrals for continuity of care.
- Use validated tools to assess identified health needs, inform decision‑making and facilitate diversion within and out of the criminal justice pathway, creating a clear individualised support plan and preparing accurate written reports in line with quality standards.
- Qualified Registered Nurse, Mental Health Nurse, Social Worker or Occupational Therapist with current registration with the relevant professional body.
- Substantial evidence of continuing professional development and post‑registration training.
- Experience working face‑to‑face with patients who have complex mental health conditions.
- Awareness of psychosocial interventions available to service users and how to refer into services.
- Awareness of substance misuse services and support available.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and will require a submission for Disclosure to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
