NHS has an exciting opportunity for a Clinical Case Manager to join their Clinical Case Management Service (CCMS) in the United Kingdom. This hybrid role requires a driver with a vehicle to travel for visits across local wards and community providers.
The Clinical Case Manager will assist in organizing service provision, manage the needs of service users and coach student nurses.
