This exciting role offers the opportunity to deliver specialist rehabilitation in patients’ homes and local communities, helping people regain independence, rebuild confidence, and achieve what matters most to them.
Working as part of a supportive multidisciplinary team, you will assess, treat, and empower individuals following stroke, using your clinical skills and creativity to support recovery and participation in everyday life. You will manage a varied caseload, work autonomously, and contribute to service development, quality improvement, and the supervision of students and support staff.
This is an excellent opportunity for an Occupational Therapist with a passion for stroke and neurological rehabilitation who is looking to develop their skills within a dynamic and rewarding community setting.

Main duties of the jobTo provide Occupational Therapy assessment to stroke and neurological patients with diverse physical and cognitive presentations, and deliver an individualised treatment programme, in patients homes and community settings.
To hold responsibility for own case load, working without direct supervision. Supervision takes the form of regular formal training and clinical reasoning sessions, peer review, case conferences and reflective practice. Access to advice and support from a senior occupational therapist is available regularly.
To undertake all aspects of clinical duties as an autonomous practitioner.
To provide expert advice, guidance and information to health and social care professionals, carers and relatives and other non-professionals.
To work as a member of the Hertfordshire Neurological Service team which specialises in the treatment and management of patients with stroke and other neurological conditions.
To be responsible for clinical supervision, education and assessment of a band five or rehabilitation assistant.
To supervise, educate and assess the performance of occupational therapy students; this would be to a graduate standard and involve working with universities to ensure the standard practice and teaching meets the standards set by the degree level qualification.
To participate in seven day working rota
To provide occasional support to the inpatient team, covering four stroke beds, providing assessment and treatment to those patients.

- To be professionally and legally accountable for all aspects of own clinical practice and work. To ensure a high standard of clinical care for the patients under your management.
- To undertake a comprehensive assessment of stroke patients and use advanced clinical reasoning skills and assessment techniques to provide patient specific treatment plans.
- To formulate and deliver an individual occupational therapy treatment programme, based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills.
- To formulate patient centred goals.
- Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
- Evaluate patient progress and treatment programmes, reassess and alter treatment programmes as required, using recognised outcome measures, assessment tools and advanced clinical skills.
- To assess and manage clinical risk within own patient case load.
- To be registered with the Health Professional Council/ Occupational Therapy register.
- To be responsible for maintaining accurate and comprehensive records in line with legal, professional and HCT’s standards of practice.
