Job overviewAre you a compassionate and experienced Registered Nurse looking to make a real difference to people with complex health needs? We are seeking a motivated CHC Clinical Assessor to join our Continuing Healthcare (CHC) and Complex Care Team.
In this rewarding role, you will support adults with complex and long-term healthcare needs by undertaking assessments, coordinating care packages, and ensuring high-quality, person-centred care. Working across health and social care systems, you will advocate for service users and their families, helping them navigate the CHC process and access the right support at the right time.
You will work closely with multidisciplinary teams, care providers, local authorities and partner organisations to deliver safe, effective and integrated care. The role also involves safeguarding, quality monitoring, risk management and contributing to service improvement initiatives.
We welcome applications from nurses with strong clinical assessment skills, a sound understanding of the National Framework for NHS Continuing Healthcare, and a passion for improving outcomes for diverse communities across North West London.
