A Care Co-ordinator plays an important role in primary care by connecting service users and carers with primary care, secondary care, community services, social care, mental health services, out-of-hours providers and voluntary organisations.

This is a non-clinical administrative role within the North PCN team. Applications are welcomed from candidates with experience or knowledge in health or social care who have strong communication skills and a genuine commitment to improving people’s lives.

The Care Co-ordinator will:

• Act as a key link between patients, carers and health and social care services.
• Support people with long-term conditions and help reduce inequalities in health and social care.
• Promote independence, choice and partnership working.
• Coordinate communication between relevant healthcare and community organisations.
• Maintain accurate IT-based information and administrative systems.
• Produce required performance information and statistics.
• Work collaboratively with healthcare professionals, social care teams and voluntary organisations.

The role is part of Joined Up Care Derbyshire
, which provides community health services to a population of more than one million people across Derbyshire and Derby.

The organisation employs more than 4,200 staff across community hospitals, health centres, clinics, GP practices, schools, care homes and patients’ homes, including through virtual consultations.
