Community Nurse

You’ll support people in their own homes through skilled nursing care, helping them maintain their health, independence and well being.

As part of a supportive District Nursing team, you will deliver high-quality care to people with a wide range of health needs. You will work closely with patients, families, carers and partner organisations to ensure care is safe, effective and tailored to individual needs. This role offers the opportunity to make a real difference every day while developing your clinical skills in a community setting.

• Carry out holistic assessments and develop person-centred care plans
• Deliver nursing care and treatment safely within your scope of practice
• Work with multidisciplinary colleagues to coordinate and improve patient care
• Maintain accurate patient records and report clinical concerns appropriately
• Support patients and carers with information, guidance and health education

• Registered Nurse qualification with current professional registration
• Understanding of community nursing and evidence-based practice
• Experience working in a healthcare environment and as part of a team
• Basic IT skills and a commitment to learning and professional development
• Ability to travel between patient visits and work flexibly to meet service needs

• Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

• Demonstrate an understanding of the role of a community nurse
• Knowledge of legislation impacting on service delivery
• Knowledge of the modernisation agenda
• Demonstrate an understanding of evidence based practice
• Knowledge of local initiatives
• Demonstrate an understanding of the MDT
