Summary:

This is a full time permanent position and hours will be worked over a 7 day rota..This role is not available for sponsorship therefore is only open to applicants who have the right to work in the UKROLE AND PURPOSE Grade 7:

To provide and support the delivery of high quality social work activities within Adult Social Care as part of a multi-disciplinary team. Works within the Vulnerable Adults operating model which includes the following teams:

Hospital See & Solve, Hospital Locality, Community See &Solve, Active Recovery Assessment, City Sensory &Telecare Support Service, East and West Locality.

The aim will be to focus on people’s strengths, agreeing goals and helping people to retain control of their life and have the confidence to maintain their independence. They will develop confidence to deliver person centred assessments and care and support plans that address people’s hopes and aspirations, as well as their physical / environmental needs, and support people to remain in control, resilient and independent.

This is a developmental role aimed at Newly Qualified Social Workers during the Assessed & Supported Year in Employment (ASYE) where additional support will be provided.

Grade 8, all of the above plus:

To provide and support high quality social work practice with increasing decision-making autonomy in situations of increasing complexity, risk, uncertainty and challenge.

Grade 9, all of the above plus:

To provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. To take a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.

There is an expectation that the social work role works within the Professional Capabilities Framework and Adult Knowledge and Skills Statements. In addition the worker at all grades practices within the Social Work England (SWE) standards.

Grade 7 ASYE level

Grade 8 Social Worker level

Grade 9 Experienced Social Worker level PRINCIPAL

ACCOUNTABILITIES :

Grade 71. To promote and safeguard the welfare of adults at risk, families and their carers, within the ‘making safeguarding personal’ principles.
2. Undertake proportionate assessments, determine eligibility, and assess risk. Plan care and support and make the best use of available resources to enable people to live the life they choose. To ensure people are given correct financial advice and information about charging processes of the council.
3. Works in partnership with Occupational Therapy and Physiotherapy utilising their expertise in the assessment and review process. To assess, identify and source low level OT equipment where this could support an individual to live independently or as part of a wider package of care. To undertake role as an Equipment Trusted Assessor.
4. Engages in critical reflection and constructive challenge within the course of working with colleagues, other agencies and with vulnerable adults and families.
5. Actively engages in team meetings/practice forums and other relevant meetings ensuring focused and purposeful social work is undertaken with all cases.
