Zorg Saam Thuis - en Ouderenzorg Zeeuws-Vlaanderen zoekt een Continuïteitsverpleegkundige voor regio Hulst. Je werkt in thuissituaties, intramurale locaties en bij urgente verpleegkundige handelingen. Je beoordeelt situaties zelfstandig en coördineert met diverse disciplines.
Je werkt zelfstandig maar staat er nooit alleen voor; collega-continuïteitsverpleegkundigen zijn beschikbaar voor overleg. Dienstverband 32 uur per week, met roosters in goed overleg.
