Key Responsibilities:

• Deliver patient education on health-related issues

• Engage with the healthcare team to coordinate care

• Maintain accurate and timely patient documentation

• Execute care plans effectively in collaboration with teammates

• Support daily functions and needs of the resident unit

Requirements:

• Required Practical Nursing diploma

• Must possess LPN license with CLPNS eligibility

• Capability to work independently and collaboratively

• Strong communication and organizational skills

• Valid driver's license may be a requirement
