Case Manager – Part-Time (Per Diem)

Location: Hummelstown, PA
Schedule: Per Diem
Compensation: $40.00 per hour

• Coordinate health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
• Coordinate care across the continuum and promote cost‑effective care within the allotted time frame.
• Assure payments of hospital‑based services meet patient‑related utilization management criteria.
• Implement safe and appropriate discharge plans; assess psychosocial needs and provide intervention as part of discharge planning.
• Primary job functions:
  Clinical Interventions/Discharge Planning;
  Care Planning Management;
  Fiscal Management;
  Payer/Referral Management.

• Current licensure per state guidelines in a clinical or related discipline OR a bachelor’s or master’s in a health or human services discipline.

• Previous experience in case management and discharge planning.
• CCM certification.

Post‑offer employment testing (POETs) are completed as part of the onboarding process and must be completed before an employee’s first day of work.
