We are looking for an Adult Support Coordinator to join our Adults Community Team in Huntingdonshire
, based in Huntingdon. This is an exciting opportunity to become part of a welcoming and dedicated locality team that is committed to supporting adults to live independently and achieve positive outcomes.

What will you be doing?

As an Adult Support Coordinator, you will play a key role in supporting adults to live as independently as possible and achieve the outcomes that are important to them.

Your responsibilities will include:

  • Undertaking Care Act and Mental Capacity Act assessments.
  • Completing reviews of existing care and support arrangements to ensure they continue to meet individuals' needs and deliver positive outcomes.
  • Developing person-centred care and support plans that promote independence and wellbeing.
  • Working collaboratively with colleagues, health professionals, and partner agencies to achieve positive outcomes.
