is urgently seeking a compassionate and skilled Hospice Admissions RN to join our dedicated team serving Orange County and South Bay LA areas. This position requires that you are available to work on Sundays 11am - 7pm with other possible per diem options throughout the week!

Must be willing to cover all of Orange County and South Bay LA areas!

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process

• Evaluate patients for hospice appropriateness, e.g. medical records review as requested.

• Obtain agreement for admission of referred patients and families who meet admission criteria.

• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Salus Hospice

• Maintain clinical and professional competence.

• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care.

• Registered Nurse with valid CA state license

• Minimum one year of hospice or home health experience

• Strong background in medical/surgical, ICU/CCU, or ER is helpful

• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient’s place of residence

• Must have excellent interpersonal skills and a passion for hospice

• Understanding of hospice philosophy and needs of the terminally ill

• Self-directed with ability to work with little supervision

• Current Physical and TB test results

• Medical/Dental/Vision - FULL TIME
