Palliative Care Nurse, Nurse Practitioner
Cabell Huntington Hospital is seeking a full‑time Nurse Practitioner for our ECCC Cancer Resource Center. This dual‑role position focuses on cancer survivorship and supportive/palliative care, working alongside the medical director of supportive care services, ECI leadership, and the cancer care team.
The Nurse Practitioner will manage an independent, comprehensive oncology survivorship clinic located in the Edwards Comprehensive Cancer Center or other Mountain Health Network facilities as needed, and will collaborate with the palliative care physician to provide integrated palliative care across the cancer continuum.
Principal Job Functions Survivorship- Provides comprehensive evidence‑based survivorship care consistent with guidelines from organizations such as ASCO and the NCI Office of Cancer Survivorship, as well as accreditation standards from ACOS and NAPBC.
- Conducts cancer surveillance for recurrence or new primaries through appropriate screening, examination, and diagnostic coordination.
- Assesses and manages long‑term and late‑term effects of cancer treatment, including physical, emotional, cognitive, and sexual health concerns.
- Provides preventative health services and education to survivors (screening, risk assessments, immunizations, etc.) focusing on wellness, nutrition, and lifestyle after a cancer diagnosis.
- Offers educational support for patients, families, health team members, and the community.
- Helps develop and maintain departmental and intradepartmental policies, procedures, and standards of care for survivorship, and provides regular reporting of program performance metrics.
- Works with physicians and other providers to effectively transition patients to the survivorship clinic and back to primary care.
- Works closely with the palliative care physician to provide comprehensive symptom management and supportive care throughout the cancer trajectory.
- Facilitates advance care planning discussions and goals‑of‑care conversations with patients and families.
- Provides psychosocial support and addresses emotional and spiritual needs of patients facing serious illness.
- Serves as a liaison between oncology teams and palliative care services to ensure seamless, patient‑centered care.
- Coordinates transitions of care between active treatment, supportive care, and end‑of‑life services as appropriate.
The Nurse Practitioner will operate within the scope of practice defined by the West Virginia Board of Nursing or the West Virginia Board of Medicine and will maintain all required licensure and certifications.
