Job Description

The Bed Planner IHR reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and regulatory and accreditation standards.

Responsibilities
  • Determine the most appropriate level of care and ensure a smooth admission experience for patients by coordinating clinical needs with operational processes and insurance benefits.
  • Use chart review to assess appropriate level of care.
  • Assign patients to appropriate units based on clinical assessment while balancing the organization’s resources.
  • Coordinate with unit charge nurse for bed placement.
  • Collaborate with nursing operations leader on throughput issues.
  • Serve as an access‑point resource for physicians, staff, and patients.
  • Respond to and assign all bed request and transfer orders in the electronic medical record.
  • Document unusual occurrences and patient relations issues; make appropriate referrals to risk management, infection control, and quality departments.
  • Utilize appropriate downtime procedures for bed placement when applicable.
  • Assist with data collection and reporting for throughput and bed placement.
  • Perform chart audits as requested by manager/director.

  • Collaborate with the admitting physician to determine the most appropriate site of care for services and any alternate levels of care available.
  • Coordinate appropriate bed placement for patients with Patient Care Services, collaborating on location, staffing, etc.
  • Utilize all available tools and resources to gain a better understanding of patients’ clinical assessment; notify physicians and staff of concerns or variations.
  • Communicate in a non‑threatening way that conveys caring and respect for patients, families, and co‑workers.
  • Communicate a complete and accurate hand‑off to the incoming shift, minimizing miscommunication errors. Complete admission forms and paperwork thoroughly.
  • Assist in easy access to incoming patients by arranging transportation when necessary, coordinating services, and notifying points of entry of patient’s impending arrival.
  • Coordinate admission with registration staff to obtain accurate patient demographics.
  • Refer appropriate cases to patient financial services, risk, peer review, infection control, and patient relations.
  • Coordinate patient activity and flow or movement between levels of care. Identify potential throughput barriers and proactively inform upper management of anticipated delays.
  • Consult with MD to discuss clinical and appropriate level of care when needed.
  • Participate in process‑improvement activities, identify opportunities.
  • Demonstrate an interest in personal growth and development beyond routine educational sessions.
  • Embrace new and existing co‑workers, contributing to their growth and development.
  • Contribute to teamwork and cohesiveness of the department and across departments.
