Job Summary:
The Assisted Living Wellness Director will foster the human experience by interacting with residents in all areas of daily life, including providing direct nursing care to residents, as well as leading the day‑to‑day nursing activities performed by the care team. You will collaborate and build strong relationships with clear communication.
- Assists in determining the type of staffing needs by client care, preparation of the team, and needs prescribed by the physician.
- Assist with weekly physician rounds.
- Provides services (ADLs and Medication Supervision) by specific written orders from a physician.
- Coordinate the clinical team in planning, coordinating, implementing, and evaluating total care within the Community.
- Records all services given on appropriate forms or Point Click Care devices.
- Reports and documents pertinent information and prepares progress notes.
- Helping the residents to maintain good personal hygiene.
- Maintains a safe work area free from hazards of fire, spills, chemicals, and equipment.
- Reports any problems to the Executive Director.
- Providing routine individualized healthcare needs.
- Monitoring or reporting changes in health status.
- Maintaining records of patient care, condition, progress, or problems to report.
- Discussing observations with the leader.
- Recording vital signs monthly such as temperature, blood pressure, pulse, or respiration rate.
- Med Administration.
- Rotating Manager on Duty.
- Nursing On-Call Rotation.
- Facilitate Team In-service Training and Mentoring of all care teams.
- Certified Licensed Practical Nurse (LPN) or Registered Nurse (RN) – Active License Required.
- Minimum 3-4 years experience working as LPN/RN in an Assisted Living Community.
