Hospice of North Alabama is seeking a passionate, dedicated Hospice Admissions RN to join our team in Huntsville, AL. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. - Provide 1:1 care to make a lasting impact on patients and families

- Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
- Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
- Obtain medical history, current medications, and symptoms
- Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
- Coordinate the start of services (nursing, aides, equipment, medications)
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation

- Benefits may vary by employment status
- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- One year of hospice nursing experience required; 3+ years preferred or similar setting such as Long-Term Care, Intensive Care, or Emergency Room considered in place of hospice nursing experience
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and in good working order
