Transitional Case Worker Mental Health

Support crisis recovery as a Transitional Case Worker in Clinton, Ontario, using assessment and crisis intervention skills. This is a temporary part-time position.

Huron Perth Healthcare Alliance seeks a Tech 5 Transitional Case Worker for its Clinton Safe Bed home. You will support individuals experiencing serious mental illness or addictions-related crises. The work includes intake, assessment, short-term crisis counselling, referrals, advocacy, and coordinated community support. You will also help maintain a safe therapeutic environment and participate in practical home operations.

• Assist intake, assessment, and support for Safe Bed clients
• Provide crisis intervention and short-term crisis counselling
• Connect clients with community resources and referrals
• Complete treatment and crisis plans with partners
• Support daily operations of the Safe Bed home

• Registered Social Service Worker or Addiction Counsellor
• Current registration and licensure with an Ontario college
• Minimum three years progressive clinical experience
• Knowledge of mental health and addiction services
• Valid Ontario driver’s license, transportation, and required insurance
