Community Health Nurse

Position Overview

The Prince George’s County Health Department is seeking a Community Health Nurse to serve in the Family Health Services Division. This entry‑level professional staff position (Grade A18‑A21, Community Health Nurse I/II) provides nursing and health care services to diverse populations—including infants, youth, adults, and seniors—across a variety of community settings such as homes, clinics, schools, and office environments.

Incumbents perform assessment, planning, implementation, and evaluation using the nursing process. They coordinate comprehensive nursing services with medical, social services, and other agencies, promoting health and wellness while preventing disease and disability. Work is performed under close supervision and evaluated through observation, record review, and employee/management conferences. Upon successful completion of a one‑year probationary period, entry‑level incumbents may be non‑competitively promoted to the II full performance level at management’s discretion.

• Provides entry‑level professional nursing services to target populations.
• Assesses health care needs of clients.
• Develops, implements, and adjusts comprehensive care plans.
• Evaluates patient status and takes appropriate action.
• Counsels patients about health care needs.
• Conducts home visits or visits in other settings.
• Administers oral medications or injections and monitors patient reactions.
• Participates in meetings, workshops, and training sessions.
• Maintains accurate, complete, and retrievable patient records.
• Performs appropriate screening tests and specimen analyses.
• Coordinates nursing services with other agencies and health care providers.

• Bachelor’s degree in nursing or related health field, or equivalent combination of education and experience.
• Valid registration as a Registered Nurse in Maryland, issued by the Maryland State Board of Nursing.
• Valid driver’s license when required for job duties.
• Current CPR certification.

• Experience with EMR systems such as EPIC.

• Meet all training and performance standards and demonstrate required proficiency.
• Wear and use agency protective apparel and equipment as needed.
• Pass pre‑employment checks, including reference checks, background investigations, and drug screenings, where applicable.
• Serve as an essential employee, reporting during standard or non‑standard hours as operations necessitate or during emergencies.
