The Prince George’s County Department of Social Services is currently seeking qualified applicants to fill a temporary Transitional Housing Rapid Rehousing Program (THRRP) Case Manager (
Community Developer I G) grade G18 position, in the Community Services D

The contractor shall perform the services set forth below under the general direction and supervision of the Prince George’s County Department of Social Services. The contractor shall serve as the Case Manager and is responsible for assisting homeless families in the Transitional Rapid Rehousing (THRRP) and HELP Program.

We do this by providing intervention services that strengthen families, protect children and vulnerable adults, encourage self-sufficiency and promote personal responsibility. We assist our customers through such programs as Temporary Cash Assistance, Food Supplement Program, Medical Assistance, Emergency Assistance, and Foster Care and Adoption Service.
Responsibilities- Complete client application on day of intake
- Read and explain resident admissions agreement on day of intake.
- Complete and place new participant's documents in case record – driver’s licenses, social security numbers, pay stubs, etc. on day of intake.
- Assist new participant in filling‑out program forms and place in case record on day of intake.
- Explain and provide new participant with program rules and regulations on day of intake.
- Take urine sample from new participant for drug testing on day of intake.
- Develop service plan, establish goals for new participant and document in case record at initial case‑management session.
- Set realistic timelines for established goals and document in case record at the initial case‑management session.
- Assess and refer new participant to job development and/or training program at initial case‑management session.
- Provide program participant with updated housing lists.
- Assist program participants in applying for services (SSI, Food Stamps, Medicaid, etc.) as needed.
- Provide ongoing monitoring to ensure families receive the services they need and are eligible for.
- Provide transportation to families for housing searches as needed.
- HMIS – Input data on program participants and update the pertinent information and service records as needed.
- Meet with program participant weekly to monitor progress and establish new goals with timelines.
- Take urine sample from program participant for drug testing bi‑weekly or as needed.
- Collect and place program participants proof of income and proof of savings in case record monthly.
- Document case management session in case record weekly.
- Monitor (weekly) participant attendance to program meetings.
- Refer and link program participant to appropriate resources weekly or as needed: housing, employment, training, clothing, food, etc.
- Contact/Identify and document established and potential resources for program participants weekly.
- Provide follow‑up on referrals and contacts the agency to ensure that services have been provided.
- Receive and distribute donations weekly or as needed.
- Assist customers in assessing the needs for their family on an ongoing basis.
- Contact local businesses and organizations to identify and secure the appropriate resources for program participants as needed.
