Molina Healthcare is seeking a highly qualified Field Nurse Practitioner to deliver comprehensive primary‑care services in homes, community settings, nursing facilities, and pop‑up clinics.
The Nurse Practitioner will work primarily in non‑clinical environments, providing care to adults, women’s health, pediatric, and geriatric patients. They will perform comprehensive health assessments, coordinate care, and collaborate with physicians and other healthcare professionals as required.
Job Duties- Provide general medical care and care coordination to adults, women’s health, pediatric, and geriatric patients.
- Conduct comprehensive evaluations including history, physical exams and preventive assessments.
- Address chronic and acute primary‑care complaints and determine medical urgency.
- Document reasonable medical diagnoses.
- Seek specialty consultations when appropriate.
- Order and perform diagnostic laboratory and radiology tests, using resources judiciously.
- Recognize when patient needs exceed scope and involve physician oversight.
- Develop and implement a medical plan of care.
- SCHEDULE patient appointments and provide post‑discharge coordination to reduce readmission and ER utilization.
- Conduct face‑to‑face visits in homes, skilled nursing facilities, public locations, and alternate modalities as needed.
- Order bulk laboratory orders to target specific member populations.
- Participate in on‑call coverage to triage urgent lab results and pharmacy inquiries.
- Attend community‑based “Pop‑Up Clinics” to build relationships and address health gaps.
- Drive up to 120 miles daily within the assigned region; extended mileage projects may require overnight stays.
- Obtain and maintain cross‑state licenses as business needs arise.
- Collaborate with fellow nurse practitioners to develop best practices.
- Participate in regional meetings.
- Prescribe medications and perform procedures as applicable.
- Maintain timely documentation in an electronic medical record system.
- Walk flights of stairs while carrying up to 50 lbs of equipment when required.
- Master’s degree in family health from an accredited nursing program.
- Advanced computer skills; proficient with Word, Excel, and Electronic Medical Record.
- An active and unrestricted national certification (American Academy of Nurse Practitioners or American Nurses Credentialing Center).
- Current state‑issued license to practice as a Family Nurse Practitioner.
- Current Basic Life Support for Healthcare Professionals certification.
Preferred Experience
- 3–5 years as a Registered Nurse and/or Nurse Practitioner in home health, community health, or public health settings.
- Experience managing chronic conditions in home health as a licensed clinician.
- Experience with underserved populations facing socioeconomic barriers.
- Fluency in a language other than English is a plus.
- Immunization and point‑of‑care testing skills.

Pay Range: $79,608 – $ ANNUAL
