SUMMARY Provide direct care to clients as described in Resident’s care plan, primarily for their activities of daily living.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Assist the client with or performs basic personal care and grooming that may include, but not limited to showering, hair care, assistance with clothing and dressing, oral care, bathroom assistance, and basic skin and nail care.

Assist to the bathroom which may include assisting with incontinent care: peri care, changing underclothes, disposing of disposable underwear, or assisting the client with a bedpan/urinal.

Assist the client with physician ordered medications. Assist with self-administration of routine eye care (caregiver must complete the Medication Assistance class and be delegated by the RN before performing this task). Ability to use a laptop for electronic medication administration documentation or willing to learn.

Assist with food, nutrition, and diet including some meal preparation, set up and clean up after meals.

Accompany client to doctor/medical appointments, assist with shopping and activities when directed by the administrator.

Assist with non-nasogastric gastrostomy tube feedings. (Requires special instructions from a RN).Assist with housekeeping tasks in the common area and in residents’ rooms which include, but are not limited to, changing bed linens, rearrange furniture, laundry, sweep, mop, vacuum, make bed, dishes, empty trash, clean refrigerator, clean small appliances, clean bathrooms, clean common area kitchen.

Assist with turning, positioning, and getting client into and out of bed.

Assist with ambulation and exercise.

Assist with transferring client with assistive devices such as gait belt, walker, and wheelchair.

Must have the ability to take, measure, and record vital signs, height, weight.

Observes and reports signs of disability, illness, and change in condition or behavior in the clients. Daily charting of task completed on progress notes.

Keeps utility areas, wheelchairs, and shower room in order.

Cleans equipment such as wheelchairs and walkers.

Able to deliver supplies, equipment, food, and correspondence to/from other areas on the campus.

Meets safety needs by reporting environmental hazards, observance of infection control measures, and oxygen safety.

Have the ability to communicate with clients, families, visitors, and staff.

Meets needs of clients with special conditions, i.e., mental illness, developmental disabilities, and behavior disorder.

Assist with activities as directed by the administrator.

Complete 8 hours of in-service per year. Must have or get CPR and first aid certified and get a food handler certification.

Assists clients with comfort and anxiety relief.

Assist in promoting client safety and environmental cleanliness.

Assists with unit management and efficiency.

Requires documentation on tasks performed for client.

CONTACTS Residents, families, other personnel, and visitors.

VARIABLES Is required to work weekends, holidays, and when called in to replace absent staff for any shift in need.

QUALIFICATIONS To perform this job successfully, an individual must be eighteen (18) years old, have a high school diploma or GED or demonstrate ability to perform each essential task satisfactorily. LANGUAGE SKILLS Have the ability to read and comprehend simple instructions, short correspondence, and memos. Be able to write simple and understandable correspondence. To communicate and effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

MATHEMATICAL SKILLSAbility to add, subtract, multiply.

REASONING ABILITYHave the ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to follow verbal and written instructions.

CERTIFICATES, LICENSES, REGISTRATIONSFBI approved Criminal Background History Check. Assistance with Medication class certificate (optional but will not be able to assist with medications if caregiver does not have this certificate). Certified Nursing Assistant must hold a valid CNA license.
