St. Luke's Health System in McCall,  seeking a Home Health Case Manager to join our Home Health team.  The Case Manager will occasionally be required to provide support to Hospice patients in the McCall area.

** Position Overview*
* As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.

** What You Can Expect in This Role*

** Full-time Schedule** :
Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts, Opportunity to work primarily on weekends and cover occasional weekday shifts as well.

Home Health and Hospice patients within the McCall service area

** Manageable Caseloads** :
Daily visit requirements are below the state and national average

** Compensation** :
Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and it's surrounding regions

** Additional Pay** :
On-call pay and applicable differential pay

** Qualifications*
* +  
** Education** : ASN or BSN in Nursing

+  
** Experience** : 2 years of RN experience;
Home Health or Acute Care experience preferred

+  
** Licenses/Certifications** :
Current RN license in the state of Idaho;
Basic Life Support (BLS) certification

** Other Requirements** :
Valid driver's license and proof of auto insurance
