Interakt Contour zoekt een Begeleider voor Poortdijk in IJsselstein voor 24–28 uur per week. Je begeleidt cliënten gericht op ontwikkeling, zelfredzaamheid en voorkomen van terugval, in samenspraak met de persoonlijk begeleider/herstelcoach voor het opstellen van het begeleidingsplan en doelen.
Je denkt mee met het dagelijks reilen en zeilen op de locatie, helpt bij koken en boodschappen en bezoekt afspraken met huisarts of externen/dagbesteding.
