Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing care services, ensuring clients receive personalised support tailored to their individual needs. This position offers an opportunity to make a meaningful difference in people's lives while working within a supportive and professional environment. Prior care coordinator experience is a MUST, as it will aid in efficiently managing administrative responsibilities associated with the role.
Duties
* Coordinate and schedule care plans for clients, ensuring timely and appropriate support.
* Liaise with healthcare professionals, clients, and their families to facilitate effective communication and service delivery.
* Maintain accurate and up-to-date records of client information, care schedules, and service documentation.
* Provide administrative support including managing appointments, preparing reports, and handling correspondence.
* Monitor client progress and adjust care plans in collaboration with relevant stakeholders.
* Ensure compliance with organisational policies and relevant health and safety regulations.
* Assist in the onboarding of new clients and staff members as required.
* Taking on the responsibility of having the on-call and ensuring all calls are answered and actioned in a professional manner.
Requirements
* Previous Care Coordinator experience is essential to effectively manage administrative tasks.
* Excellent organisational skills with the ability to prioritise tasks efficiently.
* Strong communication skills, both written and verbal, to liaise effectively with clients, families, and healthcare professionals.
* Compassionate approach with a genuine desire to support individuals in need of care.
* Ability to work independently whilst also being a team player.
* Good computer literacy, including familiarity with standard office software programmes.
* Knowledge of Birdie - Monitoring Software
* Flexibility to adapt to changing priorities within a dynamic environment.
* Able to work evening , weekdays and bank holidays
