Role OverviewAn opportunity has arisen for an experienced Band 6 Occupational Therapist to join a dynamic team delivering community-based rehabilitation for patients with a new stroke diagnosis. The post holder will manage a caseload, conducting assessments and delivering treatments that support recovery and rehabilitation, in line with national stroke rehabilitation guidelines. This role offers an autonomous clinical environment, working within a multidisciplinary team (MDT) to ensure patient-centred care and optimal rehabilitation.
Key Responsibilities- Manage a caseload of patients with new stroke diagnoses within the community setting.
- Complete comprehensive assessments of patients' functional needs and develop individualised treatment plans.
- Provide evidence-based rehabilitation interventions, working towards the restoration of independence and improved quality of life.
- Demonstrate a thorough knowledge of current stroke rehabilitation guidelines and incorporate them into treatment plans.
- Order appropriate equipment to optimise patients' independence and improve daily functioning.
- Work collaboratively within an MDT to achieve optimal outcomes for patients.
- Communicate effectively with patients, families, and colleagues, ensuring a patient-centred approach to care.
- Maintain accurate and timely clinical documentation in line with HCPC regulations.
- Degree in Occupational Therapy and HCPC registration.
- Significant post‑qualification experience in community rehabilitation, particularly in stroke care.
- Demonstrated ability to work independently and manage a caseload effectivelyన
- Strong communication skills, including experience in joint working within multidisciplinary teams.
- Knowledge and experience in ordering appropriate rehabil Besonders equipment.
- Experience working within NHS community rehabilitation teams.
- Experience with stroke‑specific rehabilitation guidelines.
- Proficiency in using electronic patient record systems.
- Enhanced DBS
