Position: Advanced Level Pharmacist - Crisis Pathway and Home Treatment Team

Advanced Level Pharmacist – Crisis Pathway and Home Treatment Team Overview

We have a vacancy for an Advanced Level Pharmacist who will provide and develop patient‑centred clinical pharmacy services to the Crisis Service and Home Treatment Team within a multidisciplinary setting. The successful applicant will act as a role model for Junior Pharmacists undertaking their Clinical Diploma, providing support and supervision.

Key Responsibilities
  • Provide expert pharmacy and medicines management advice to optimise patients’ medication regimes.
  • Prescribe medicines as part of the multidisciplinary team; non‑medical prescriber status is essential.
  • Support the development of teams involved in the Crisis Pathway, Home Treatment Teams (HTTs), Acute Crisis and Assessment Teams, and the integrated Care Hub.
  • Supervise and support Junior Pharmacists and clinical trainees.
  • Contribute to the provision of clinical supervision to develop further expertise in Adult Mental Health.
  • Collaborate with the clinical team to facilitate safe discharge and transition back into primary care.
Qualifications & Experience
  • Clinical experience in mental health medicines optimisation.
  • Non‑medical prescriber training and qualification.
  • Ability to provide expert pharmacy and medicines management advice within adult mental health settings.
  • Strong communication and teamwork skills within a multidisciplinary team.
